Diabetes Management
Our Livongo diabetes management program includes support to help you manage diabetes, pre-diabetes, high-blood pressure, and other health goals including managing your weight and stress. The program is available at no cost to medical plan members diagnosed with diabetes. Receive a smart touchscreen glucose meter that automatically uploads readings and provides personalized tips, as well as unlimited test strips and 24/7 support from coaches. Fertility
To support all types of growing families, PGTI offers up to a total of $35,000 maximum lifetime coverage ($25,000 for medical services and $10,000 for prescription services) for infertility treatment, including evaluation, drug therapy, in vitro fertilization (IVF) and egg freezing. In order to be eligible for these benefits, you must enroll in a UHC medical plan. We encourage you to seek services from in-network providers due to some services requiring prior authorization.

Tobacco Cessation
The Quit4Life program through UnitedHealthcare makes it easier to quit smoking and start living tobacco free. And it’s available at no cost to you if you’re enrolled in a UnitedHealthcare medical plan. Best of all, you can lower your medical premiums if you’ve been paying the tobacco rates.

The Employee Assistance Program (EAP), provided by Optum EAP, is a free benefit to help you and your family members live well, at home and at work. The EAP offers:

- Up to three face-to-face or virtual sessions per issue at no cost to you
